Terms and Limitations of the Smart Dental Discount Plan


  • This is a dental discount plan and is not dental insurance. An upfront membership fee is required to join.
  • This plan cannot be combined with any other dental insurance. This is due to legal restrictions.
  • This Plan is good ONLY at Keith P. Rojek DDS PC. Therefore, if you are referred to a specialist, they will not offer this discount.
  • Family members cannot be substituted in for another family member, each must be enrolled.
  • Your effective date is the day you sign up and your renewal date is exactly the same date every year.
  • Cleanings must be performed before the end coverage date, there are no roll over of benefits.
  • Should there be any dental treatment needed following any type of injury where a lawsuit and therefore outside medical, auto, disability, or workman's comp type of insurances are involved, this discount plan cannot be used.
  • There is no discount on products purchased through our office.
  • This plan is non refundable and non transferable. No refunds will be given if patient(s) chooses not to use their dental plan.
  • Plan is subject to change yearly, rates and fees are subject to change annually



Some more "Rules"......

All payments are due AT TIME OF SERVICE to receive the discount. Any services that are received that are not paid for at the time of service will be billed at the usual, higher fee.

This offer cannot be combined with any other offers or discounts. Plan fees are valid only when paid at time of enrollment.

Primary Patient must remain a member during duration of dental treatment of other family members. Additional members under primary are eligible only through the duration of the primary. Therefore for best value, all members should be enrolled together. Should extensive work be required on secondary patient, over term of primary, the choice would be to enroll that person as primary, or renew the existing primary.

Family members are limited to spouses and children. Children must be under age of 24.
